2024/25 FLU SEASON  The vaccination programme starts later this year than in previous years. Pregnant ladies will be invited to have their vaccination in September. All other age groups can be vaccinated from 3rd October. Our flu clinic at the Scout Hut in Bere Regis will be on Saturday 19th October. Please put the date in your diary if you are eligible for this vaccine!  We will advise you when we are taking bookings for our clinics.

Accountable GP

All practices are required to provide all their patients with a named GP who will have overall responsibility for the care and support that our surgery provides to them.  Your registered GP will be your accountable GP.  This does not prevent you from seeing any GP in the practice.

REGISTRATION WITH THE PRACTICE.

You can call into the practice and complete a registration form, alternatively you may print off a registration form from the link below.  Please fill it out and bring it in with you to the practice.
PLEASE NOTE, WE REQUIRE PHOTOGRAPHIC ID (DRIVING LICENCE, PASSPORT, BUS PASS) FOR EACH ADULT, IN PERSON, REGISTERING AT THE PRACTICE.  IF YOU HAVEN’T ALREADY FILLED OUT A NEW PATIENT QUESTIONNAIRE YOU WILL BE ASKED TO DO SO AT THIS STAGE.

NEW PATIENT QUESTIONNAIRE
When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.
NEW PATIENT HEALTH CHECK
When you register you will be offered a new patient health check appointment with one of our Health Care Assistants.  This is a 20 minute appointment where the healthcare professional will carry out a few simple tests to assess your risk of heart disease, diabetes, kidney disease and stroke.  Your weight and height will be measured, your blood pressure taken and your cholesterol checked.  You may be offered diet and lifestyle advice.  If you smoke you will be advised about smoke stop services available.
